Insurance & No Surprises Act

Insurance Information

Foxhole Health & Consulting contracts with multiple insurance carriers; however, participation with a specific insurance company does not guarantee that we are in-network with every plan offered by that carrier. Before scheduling your first appointment, please contact your insurance company to verify:

  • Is Foxhole Health & Consulting is in-network with your plan?
  • Is Kim Cafaro, ARNP is in-network with your plan?
  • Is a prior authorization required​?

Out-of-network services may result in higher deductibles, co-pays, co-insurance, or claim denials. It is your responsibility to ensure that your insurance coverage is active at the time of service. If coverage lapses or services are denied by your insurance company, you may be responsible for the full cost of services.

No Surprises Act

The No Surprises Act protects individuals with health insurance and self-pay clients from unexpected medical bills for certain services. Upon request, Foxhole Health & Consulting will provide a Good Faith Estimate outlining anticipated costs for services provided. If you receive a bill that is more than $400 above your Good Faith Estimate, you may have the right to dispute the charges.
